If it has been sitting for a long time, it must be shaken thoroughly...

...and many times, prior to use. I have revived some old small Duplicolor cans recently (one had a $.59 price tag from a closeout store; it's been a long time since they were that cheap).
First stage would involve several minutes of shaking. Next step is to roll the bottom of the can, to stir up everything that has settled to the bottom. When you are done, the agitator inside the can should move freely around the bottom of the can, with no resistance. More shaking follows in the days prior to use.
On the day of use, shake according to the instructions on the can, as by then the contents should be revived to a point where it should be equal to a new can.
I would do this with a recently purchased can also; after all, you don't know how long it was sitting on the store shelf, and in the warehouse prior to that...
